A survey for 5th grade students before starting a classroom guidance unit on the transition to middle school. This needs assessment informed which aspects of the transition to address. The most frequent question topics included: having/how to open a locker, making schedules/managing homework, and choosing electives. The scale at the bottom was also administered in a post-test after the guidance unit, identifying students who remained anxious about the transition. These students will receive small-group or individual counseling to help them feel more prepared to transition to middle school.
